bacterial (typhoid fever, miliary tuberculosis)

protozoal (visceral leishmaniasis, malaria)

autoimmune disease:

rheumatoid arthritis with Felty's syndrome

systemic lupus erythematosus

idiopathic

other causes:

racial (African and Middle East populations)

familial

cyclical neutropaenia

selective lymphopaenia

infections:

typhoid

brucellosis

other causes:

inherited (rare)

small intestinal lymphangiectasia [+ lymphopoenia + low Ig]

radiation
