Hashimoto's thyroiditis [commonest cause; goitre present]

spontaneous atrophy of gland [non-goitrous]

treated hyperthyroidism:

carbimazole

thyroidectomy

radio-iodine

radiation

goitrogens:

iodine

amiodarone

PAS

lithium

other causes:

hypopituitarism [low TSH, high TRH]

hypothalamic disease [low TRH and TSH]

endemic iodine deficiency (dietary lack)

1° agenesis of gland / congenital hypothyroidism [causes cretinism]

peripheral resistance to thyroid hormone

inborn errors of thyroid hormone synthesis:

dyshormonogenesis (various), eg. Pendred syndrome (+ deafness)
