pituitary TSH adenoma [<1%; may arise following prolonged hypothyroidism]

factitious [self-administration of thyroxine]

subacute or chronic thyroiditis (usually cause hypothyroidism)

amiodarone

neonatal hyperthyroidism (infant of mother having Grave's disease)
