primary or tertiary hyperparathyroidism [high Ca, low P; not suppressed by steroids]

hyperthyroidism

Addison's disease

phaeochromocytoma

infantile hypothyroidism

malignancy:

bone metastases [commonest cause in hospitals; usually suppressed by steroids]

multiple myeloma

paraneoplastic syndrome [ectopic PTH or PTH-like hormones; especially squamous cell ca of lung]

renal:

recovery phase of acute tubular necrosis

drugs:

thiazide diuretics [calcium retention by kidneys]

milk-alkali syndrome

other / miscellaneous:

prolonged immobilisation, especially in Paget's disease

hypervitaminosis D

sarcoidosis [also high vitamin D levels]

idiopathic hypercalcaemia of infancy
