cardiomyopathy

left bundle branch block (LBBB)

Wolf-Parkinson-White syndrome

non-cardiac:

anaemia

myxoedema

hypokalaemia

hyperventilation

hypertension, esp. if 'LV strain' present

digoxin or beta-blocker treatment

pre-menopausal female
