dysthyroid eye disease [Grave's disease; commonest cause of unilateral or bilateral exophthalmos]

SVC obstruction

severe myopia [large eyeball]

Cushing's syndrome

uraemia

malignant hypertension

craniostenosis [congenital]

familial

unilateral exophthalmos:

carotico-cavernous fistula [pulsatile exophthalmos]

cavernous sinus thrombosis

retrobulbar haemorrhage

orbital cellulitis or periostitis

buphthalmos [large eyeball due to congenital glaucoma]

orbital masses:

tumours / metastases

cavernous haemangioma

lachrymal gland tumours

optic nerve glioma or meningioma

granulomas, eg. sarcoidosis, tuberculosis
