adrenal or parathyroid disorders

hyperthyroidism

pregnancy

previous head injury

dural venous sinus thrombosis [an important differential, rather than a cause]

drugs:

tetracyclines

nalidixic acid

nitrofurantoin

steroids (treatment with or withdrawal of)

sex hormones

oral contraceptive pill

tamoxifen

vitamin A and analogues (e.g. retinoids)

vitamin D

lithium

thyroxine

growth hormone

amiodarone

sulphonamides
