INVITATION  TO  ACCREDIT  AND TO OFFER

The Department of Agriculture, Regional Field Unit No. 6 Iloilo City invites all accredited suppliers/manufacturers /distributors to participate  in the negotiation for the Procurement of  Four (4) units Screenhouse  : ABC P265,000.00  with the following description

4   unit  Screenhouse with the following specifications and components

            -           3.3m x 2.3m x 9m dimension

            -           Four (4) nursery benches with stand

            -           20 seedling trays (104 holes, PP, UV treated)

            -           UV treated screen nets

           -           Mobilization and installation charges included

                                                3 ft. width   x 6 ft length x 2 ft height

                                                1/2 inch diameter GI pipe frame

                                                7 cm x 3 cm mesh eyelet, 2 cm x 1 cm thick

                                                1/2 inch GI pipe frame assembly using hard alloy steel clips

Interested suppliers/manufacturers/distributors must submit requirements for accreditation in order to participate in the negotiation for said procurement.

Deadline for submission of documents for accreditation and offer is on or before 5:00 P.M. ( August  25, 2009)

REQUIREMENTS   FOR ACCREDITATION

1. Company  Profile
2. DTI  Registration (Sole Proprietorship )

SEC Registration (Corporation)

3. License to Operate (Mayor’s Permit)
4. ITR for the last  two years (stamped received by BIR)
5. Audited Financial Statement for the last two years
6. VAT/E-VAT/NON-VAT Registration
7. Number of Employees/Organizational Chart (certified by HRD Manager)
8. List of Clients with their respective telephone number and addresses
9. The company must indicate in its application the category/classification/grouping they would like to participate in (AMDA)
10. Certification that the Company is not affiliated with other companies with similar nature
11. Listing of products/services offered.
12. Certification of dealership from authorized distributor
13.  In case of being an exclusive distributor Certificate  of Sole Distributorship
14. Sworn Statement that each of the documents submitted is an original or true and faithful copy of the original or must be duly notarized.

  Note :  Items No. 9,  12, and 13  are optional requirements.
