INVITATION TO ACCREDIT  AND TO OFFER

 

The Department of Agriculture , Regional Field Unit No. 6 Iloilo City invites all accredited suppliers/manufacturers /distributors to participate  in the negotiation for the Procurement of  500 heads Gilts  (ABC P5,500,000.00) with the following description-

 

Crosses of Large White Landrace and Duroc Jersey, not less than 60 kgs. bodyweight, at least 4 mos. old, dewormed, vaccinated with Hog Cholera, with good body conformation, apparently healthy without physical defects and fit for breeding  purposes.

 

To be delivered in the drop-off points in the municipalities of  the  different provinces .

                        Terms of  Reference  :

 

1.      All stocks shall be sourced from a BAI  Registered  Piggery Farm.

2.      Stocks for delivery shall be pre-selected/inspected, weighed and marked properly in the farm 1-2 days before the scheduled delivery.

3.      Final inspection shall be done at the drop-off points.  Distribution of said animals shall be at the identified place in the municipality

4.      Stocks found to be defective/sick shall be replaced within 7 days after delivery provided that the cause of death/defect is not due to negligence of the beneficiary.

5.      Delivery period should be completed within 3 months .  Mode of payment shall be based on complete delivery per province or for every 100 heads of delivery.

6.      All stocks for delivery shall be certified by a licensed veterinarian that the stocks have been dewormed, vaccinated with Hog Cholera, healthy with no physical defects and fit for breeding purposes.

7.      Specifications-based on the Purchase Request

                    

Other interested suppliers/manufacturers/distributors must submit requirements for accreditation in order to participate in the negotiation for said procurement.

 

Deadline for submission of documents for accreditation and offer is on or before  5:00 P.M.  (  May 29, 2009) .

 

REQUIREMENTS  FOR ACCREDITATION

 

  1. Company  Profile
  2. DTI  Registration (Sole Proprietorship )

SEC Registration (Corporation)

  1. License to Operate (Mayor’s Permit)
  2. ITR for the last  two years (stamped received by BIR)
  3. Audited Financial Statement for the last two years
  4. VAT/E-VAT/NON-VAT Registration
  5. Number of Employees/Organizational Chart (certified by HRD Manager)
  6. List of Clients with their respective telephone number and addresses
  7. The company must indicate in its application the category/classification/grouping they would like to participate in (AMDA)
  8. Certification that the Company is not affiliated with other companies with similar nature
  9. Listing of products/services offered.
  10. Certification of dealership from authorized distributor
  11. Certificate of Sole Distributorship ( in case of being an exclusive distributor)
  12. Registration as BAI  registered  Piggery Farm
  13. Sworn Statement that each of the documents submitted is an original or true and faithful copy of the original or must be duly notarized.
