Soyuz TM-16 spacecraft (Manakov and Poleschuk, 1992). This Soyuz was the first spacecraft to test the APAS androgynous docking system, later used by the american shuttle in the joint docking missions with the Mir space station.

Soyuz 1 crash. Vladimir Komarov died when his capsule crashed due to an incorrect parachute deployment (the secondary parachute became tangled with the main one), after the first Soyuz spacecraft manned mission (1967).
