participation in this bike ride.  I represent
that I am in proper physical condition to
participate in this ride.  For my safety, I also
agree to wear a helmet.  I grant full permission
for the use of my photographs for any publicity
and/or promotional purpose without obligation or
libality to me.  If entrant is under 18 years of
age, parent or guardian must sign entry.  Children
under 12 must be accompanied by an adult at all times.
