| Submit a voucher to receive reimbursement for union-related expenses and/or payment for wages lost while conducting union business. The form below will automatically total your dollar amounts as you type them in. Simply click on or tab over to the pertinent fields, type in your entries, and then print the voucher. Make certain to sign the voucher and include all necessary original receipts, including original charge card slips. Write the reason for each expense on the voucher and/or receipt. . |
| DON'T FORGET: MACKINAC BRIDGE FARE RECEIPTS! Reimbursement for bridge fare requires receipts. Get receipts at the fare booth when crossing the Mackinac Bridge. |

| W-4: REQUIRED |
| In order to receive compensation from the Local for wages lost while doing union business, you will need to submit a federal W-4 tax form and a Michigan state W-4. These forms specify your tax status and total exemptions. You may also submit these forms to replace any filed by you previously. |
