UUCA-CUUPS Membership Information Sheet We're glad you're interested in joining the UUCA chapter of CUUPS! Please fill out the appropriate registration form (individual, family, or minor without parent attending) and submit it along with the correct amount of dues to: CUUPS Registration C/O UUCA 1911 Cliff Valley Way NE Atlanta, GA 30329. Please make checks payable to UUCA and write CUUPS in the memo space. Below you will find information to help you determine which form to use and how much dues to send. Family: A family is considered a group of two or more people related by marriage and co-housing together or non-married partners co-housing together. If it would be cheaper to sign up individually, you are not required to sign up as a family. Minor: Anyone under the age of 18, whose parent(s)/guardian is(are) not interested in becoming members must have express written permission from a parent or legal guardian. Individual: This is the category that all others fall under. Dues Individual $10/year Family $25/year Junior (10yrs.-17yrs.) $ 5/year Student $ 5/year Retiree $ 5/year If you are unable to afford dues, speak with a councilperson to make arrangements. Please keep in mind that membership in UUCA-CUUPS does not include membership in National CUUPS (which is highly recommended for those who are not already members). Information on becoming a National CUUPS member can be found on their website at . Membership in UUCA-CUUPS gives voting privileges for one year from receipt of dues. Children under 10 years of age do not pay dues and do not have voting privileges. Minors who are not members and wish to attend a CUUPS function without a parent or legal guardian also attending must bring a permission waiver signed by a parent or legal guardian. The waiver can be found on our website at . If your child plans to bring a friend, please print a form off for the friend to take home or ask their parent/guardian to print one off to sign prior to the event.
