CLINICAL CASE PRESENTATION

Hip distraction with Ilizarov hybrid frame followed by total hip replacement for the treatment of late neglected congenital dislocation of the hip (2002).

 

This 45 yo lady had CDH, which was not treated. She presents a 6 cm shortening, functional impairment and pain on walking.     

 

A hybrid frame was built with components of Ilizarov and monolateral external fixator. After a Girdlestone procedure we have applied the frame with 4 pins on the supra acetabular zone, anterior and posterior to the greater trochanter 4 pins on the femur. Distraction was begun on a ambulatory base, at a rate of 2 mm/day as tolerated by the patient.

 

One month after, we have discarded the frame and the patient remained with traction on bed for 3 weeks. Then she was subjected to a THR, utilizing a Corail steam with ceramic-ceramic interface. 

 

At the 10 month follow up control, patient was walking with one crutch, has a stable implant, 85� active flexion of the hip, improved abduction. No infection or sciatic problems.
