I am writing to request employment-related documentation as your
former employee. As you will recall, I
was in your employ as a live-in caregiver from [start date] to [end date].
I have repeatedly attempted to obtain my T-4 slip (tax year
20xx) and Record of Employment (R.O.E.) from you, without success. As you are no doubt aware, you are required
to complete an R.O.E. within five calendar days of an employee’s final day of
work. In addition, two copies of an
employee’s T-4 slip must be provided by the final day in February following the
calendar year to which the slip applies.
Both of these documents are now late.
The Canada Revenue Agency advises that there are substantial penalties
including fines and/or imprisonment for failing to provide an employee’s T-4
and Record of Employment.
This letter shall serve as a final request to produce my T-4
slip (tax year 20xx) and Record of Employment.
Please have both of the aforementioned documents completed and available
