YOU ARE HEREBY NOTIFIED TO APPEAR IN THE ABOVE NAMED COURT
ON:
February 6, 2006 at 10:00 A.M.

NOTICE: You may mall or bring the cash deposit o[ $500.00 to Columbia County
Clerk of Courts at P.O. Box 587, 400 DeWitt Street, Portage, Wisconsin 53901
on or before February 6, 2006

If you make such cash derposit, you are not required to appear in court unless
summoned. By making a deposit and not appearing in court, you will be deemed
to have entered a plea of no contest and submitted to forfeiture not exceeding
the amount of the deposit.
If the court does not accept your plea of no contest, you will be summoned to
appear in court. IF YOU DO NOT MAKE A CASH DEPOSIT AND DO NOT
APPEAR IN COURT AT THE TIME ABOVE SPECIFIED AN ACTION MAY BE
COMMENCED AGAINST YOU TO COLLECT ThE FORFEITURE AND COST.
