Membership application form Fill in this application form if you want to join the Usher Group. Directly to Membership Coordinator or indirectly through Group Leader(s).
2004 Membership Renewal form Submit this form, along with a $2 membership fee to keep your information on the database. Directly to Membership Coordinator or indirectly through Group Leader(s).
Change of info notice If you have any change in info, please fill out this form. Directly to Membership Coordinator or indirectly through Group Leader(s).
Reference letter application If you need the Church to certify your volunteer work, you have to fill this form out, along with the standard church application form. Please note that members who have joined after August 16, 2002 will not be certified regarding community hours (other certification will still be issued). Your Group Leader.
