For
reservation , kindly provide name according to passport / passport number / date of birth / sex /
nationality.
Please remember to provide your home / mobile telephone numbers for urgent

Always check your reservation by fax or email
and ensure name written is according to your passport. Don't wait till ticket
is issued, it's too late to amend.
Take note name appear as shown on your flight itinerary will be auto

Please be reminded that it is mandatory that all PNRs for United Airlines flights contain the complete first, last and middle names of the passenger followed by title (such as MR, MRS, MS, MSTR). No initials will be permitted for any part of the passenger name.
Failure to comply with the above may result in the customer being denied boarding on any United flight.

Visa policy for entering Semarang, Indonesia.
Wef from 31st August 2004, ACHMAD
YANI airport in SEMARANG has been declared as an International Airport. As
such, visitors from the 11 countries (see attached example ASEAN countries) do
not require VISA for entry into SEMARANG.
Facilities for those visitors
intending to apply Visa-On Arrival is still under process, hence they are
required to process the necessary VISA at the Indonesian Embassy prior to
departure to SEMARANG.

Passengers, travelling to/from
Semarang on our direct services, ie GA875 and GA874 must possess valid VISA
prior to their departure. Please be informed that VISA facility on arrival is
not permitted.
Alternatively, passengers can opt for flights to Semarang
via Jakarta for those who do not possess a valid VISA
Visa is exempted only for Indonesian passport holders with
valid KIMS or KITAS documents.
No show fee
If you are unable to travel, kindly ensure all bookings are cancelled prior to
departure to avoid SGD150 no show fee.
RELOCATION OF SHUTTLE TICKET SALES COUNTER
Please note that with immediate effect, the sales counter for shuttle tickets to Kuala Lumpur at Passenger Terminal Building 2 (PTB2) has been relocated to the main SQ Ticket Office, located at #026-023/024 in PTB2.
Airport
check in
You are advised to check-in at least 2 hours before the time of departure as
some airline closes check in counter 60 minutes before departure.
With effect from 01 August 2004,
Changi Airport Terminal 2 will close at 30 minutes prior to the
scheduled departure time on all Singapore to Kuala Lumpur flights.
All shuttle ticket passengers must report not later than 30 minutes at our check-in counter before the scheduled departure time.

To provide greater flight punctuality to our passengers, we will be closing our check-in and boarding for all SIA flights departing from Singapore earlier. This will take effect on 15 January 2005.
All SIA flights departing from Singapore will stop accepting passengers for check-in at 40 minutes to departure time, even if there is a confirmed reservation.
Boarding gates will close 10 minutes before the flights’ scheduled time of departure. Passengers who are not at the boarding gate when the gate closes will be denied boarding and offloaded.
Flight Reconfirmation
Some airlines still require reconfirmation at least 72 hours before departure.
Take note all China carriers require flight reconfirmation or seat will be auto
cancel.
TICKETING DATELINE
We
would like to reiterate once again that all ticketing deadlines must be
strictly observed. With effect from 3
November 2004, we will not entertain any late issuance that does not conform to
the ticketing deadline. Bookings that
have exceeded the deadline without ticket numbers will be auto-cancelled
without notice.
With effect from 3 Aug 2004, passengers can check in for
selected Singapore Airlines (SIA) flights using text messages. With
the introduction of the SIA Short Message Service (SMS) Check-in, frequent
flyers (PPS Club and KrisFlyer members) and registered users of the SIA website
can conveniently check-in using their mobile phones.
Selected cities are Auckland, Bandar Seri Begawan, Bangkok, Chennai, Christchurch, Dhaka, Hanoi, Kuala Lumpur, Manila, Penang, Singapore and Taipei.
To access the
service, customers must first register at the SIA website www.singaporeair.com. At the website, go to “My Flight Alerts” and
click on “Check-in Notification Service via SMS”. On checking in via SMS on the
mobile phone, the customer will receive a confirmation of the check-in and the
allocated seat number also via SMS.
On
the day of departure, customers need only go to the airport to pick up their
boarding passes and check-in their baggage, therefore saving time standing in
line.

EXCESS
BAGGAGE REVISION
For travel
originating on/after November 1, 2004, Northwest Airlines will reduce the free
weight allowance for international checked baggage from 70 pounds (32
kilograms) to 50 pounds (23 kilograms) per piece on Economy class.
World Business class customers may continue to check up up to 70 pounds (32
kilograms) per piece.
An overweight fee
in the amount of $35USD/$50CAD will be assessed for bags that weigh more than
50 pounds (23 kilograms) and up to 70 pounds. Bags that weigh between 71 pounds
(33 kilograms) and 100 pounds (46 kilograms) are subject to the current fee
calculation based on excess baggage rates for the applicable origin and
destination. As a standard, baggage that weighs over 100 pounds must be sent
via alternate means using a known shipper or freight forwarder.
Effective 01 November 2004, the maximum permitted weight per piece of checked baggage for all flights to / from / via Great Britain is 32 kg.
Any piece of baggage that is heavier shall not be accepted for carriage. Nevertheless, the free baggage allowance on all classes of service remains the same
SOUTH AFRICA-ENTRY REQUIREMENT
Kindly note that the South African Immigration requires one
empty page or one unused page face available in all foreigners passports for
entry into South Africa.
A fine of ZAR5000 will be given in the event of non adherence to this
regulation.

US-VISIT
Program
Effective September 30,
2004, visitors to the United States who travel under
the Visa Waiver Program
(VWP) will be subject to the US-VISIT (Visitor and
Immigration Status Indicator
Technology) program. This means they
will be
photographed and
electronically finger scanned upon arrival.
On this date,
all visitors to the U.S.
between the ages of 14 and 79, except nationals of
Canada, must register under
the US-VISIT program. Travelers who
require a visa for travel to the United States have been subject to US-VISIT
since the beginning of 2004. The
process for departing visitors will be phased in at a later time.

Visa Waiver Program (VWP)
Effective October 26, 2004,
each passenger who wishes to enter the U.S.
under the VWP must have a
machine-readable passport*. Children, including
infants, must also have their own machine-readable passport when traveling
under the VWP. Nationals of VWP
countries remain subject to US-VISIT even when holding a machine-readable
passport. When the passport does not
contain a machine-readable zone, nationals of the VWP countries require a valid
U.S. visa (all visa holders between the ages of 14 and 79 are subject to
US-VISIT).

Required
Use of Machine-Readable Passport (MRP) for Visa Waiver Program Travel
With effect
from 1st October, 2003, travelers entering the United
States under the Visa Waiver Program must have a machine-readable
passport.
Any traveler without a machine-readable passport will be required to obtain a
visa before coming to the United States.
Immigration
inspectors will deny entry to any traveler attempting to enter on a
visa-waiver basis without a machine-readable passport after October 1,
2003. It applies to both adults and children must have individual
Machine-readable passport.
Airlines will
therefore be checking that all passengers’ passports are machine-readable
and children or babies have their own separate passports. Passengers who do not
comply will be offloaded.

All
passengers travelling into South Africa must have at least one blank page in
their passports. South Africans are
exempted. Passengers must also hold onward/return tickets.
A yellow fever
vaccination certificate is required from travellers over one year of age who,
within the preceding six days have been in or have passed through any country
partly or wholly endemic for yellow fever. This is the current vaccination
certificate requirement for entry into Singapore.

We would
therefore appreciate it if you could initiate action to ensure that passengers
and crew intending to enter Singapore, within six days after visiting or leaving
a yellow fever endemic country, are duly vaccinated. They would be required to
produce the relevant International Certificate of Vaccination against yellow
fever at the immigration check-in counter.
A passenger
who does not have a valid yellow fever vaccination certificate and refuses to
be vaccinated at the airport would be treated as a not-to-land (NTL) case. In
such a case, the passenger would have to be deported back to the port of
origin.
Measles Vaccination requirement
Advise by
Continental Airlines, with immediate effect, all passengers 15 years and under,
originating from Majuro must provide proof of measles vaccination. If a
passenger does not have a proof of vaccination, they will be referred to
the local Public Health Clinic in Majuro and will not be permitted to board our
flight.
Silk Air no longer serve meals for Langkawi, Padang and Palembang traffic on economy class.
