Vendor Registration Form Cincy/NKY PPD Vendor Registration Form All Information must be completely filled out I, ___________________, of said business, _________________________, understand and acknowledge that by signing this form, I release Pagan Pride Day from any and all responsible for any of the following; theft, weather damage, low sales, injury, breakage of any merchandize or any other incidents that may occur while vending at the Cincy/NKY Pagan Pride Day Event.

I understand that I will pay a small fee of $20.00 plus a donation for the fundraising auction for a vending space at the Cincy/NKY Pagan Pride Day event. I understand that it is my responsibility to abide by all local and state tax laws; I understand I am responsible for all necessary permits needed for vending.

I understand that I am responsible for providing my own tables, tarps, tent and/or shelter.

In exchange for the amount paid, Cincy/NKY Pagan Pride Day will provided me with an area to vend my wares, plus a listing in the vendors list on the website, flyers and other sources of media they may use.

I also understand if I fill this form out and have it turned back in with payment by and no later than August 30, 2002, the fee for vending space will only be $20.00. After August 31, 2002 the cost for vending at Cincy/NKY Pagan Pride Day Event will cost $25.00
