As a client of Christina Hall, I understand and agree to each of the following provisions. Please initial after each item.

1. All items included in this project are to be mounted in an archival-quality album using only photo-safe adhesives, papers, accents, pens, etc.  I understand that only those products labeled by reputable companies as �archival-quality�, �photo-safe�, �acid-free�, and/or �lignin free� will be used, and thus any resulting damage from defective products are not the liability of Christina Hall.  I also understand that if I request the use of items that may not be of archival quality, these items may cause damage to my photos [personal memorabilia, etc.]. ____________

2. I understand that photos and memorabilia will be mounted using permanent and archival safe adhesive, unless otherwise specified. I also understand that removal of photos is not recommended, as it may damage the photos. Any photos or memorabilia that are not to be mounted permanently are noted on the inventory sheet. These items will be adhered with photo corners. ____________

3. I understand that it is my choice to provide Christina Hall with either original or duplicate photos. In the event that irreplaceable photos are damaged, I accept full responsibility and will not hold Christina Hall liable. I also understand that should the photos be damaged in the event of a natural disaster or other unavoidable circumstance (theft, fire, car accident, etc.), it is my responsibility and I will not hold Christina Hall accountable. ____________

4. I understand that photos may be cut (�cropped�), and that any photos I do not want cropped are indicated on the inventory sheet. ____________

5. I understand that Christina Hall requires a non-refundable deposit, 50% of the project cost, at the time this agreement is signed. I also understand the remaining balance will be paid upon delivery of the album. Additional charges may include photo organization, printing of photos, maine tax, and shipping/handling.  Christina Hall accepts the following forms of payment: personal check, money order, or cash (no cash through the mail, only in person)____________

6. When the project is finished, I understand that I will be notified, and the remaining balance will be due at that time. I will provide the balance in full when the project is delivered. If it is being shipped, I will provide the balance due prior to shipping. ____________

7. If the project is to be shipped, Christina Hall will ship it through a reputable shipping agent. I understand that it will be insured, with proof of delivery required.  I also understand that Christina Hall is only responsible for the shipping insurance. Any loss or damage that occurs will be the liability of the shipping agent. Any shipping expenses incurred by Christina Hall will be added to my account and included in the �balance due.� ____________

8. I will have 7 days from the initial confirmation of receipt to notify Christina Hall if there are any corrections to be made.  I understand that acceptance of the album is acceptance and satisfaction of all work completed. ____________

9. I understand that I cannot return a completed project and get a refund, as this is custom made for me. ____________

10. I understand that Christina Hall will make every effort possible to complete this album on the date indicated in this agreement. If it is not ready by that date, Christina Hall will give you a 10% discount on the total balance. ____________

11. I understand that all returned checks will be subject to a $30 inconvenience fee and work on the project will stop until payment is made.  If the payment is not re-made within seven days, returned checks will be forwarded to collection authorities. ____________

12. Please initial only one of the following two choices:
I agree that Christina Hall can make and keep copies of my particular project to be used for promotional purposes, including but not limited to Christina Hall�s demo album, brochures, advertising, etc.  ____________
I would prefer the design work involving my photos and memorabilia  remain private and confidential. Please do not make copies and/or use any design work for any advertising or promotional purposes. ____________

13. If I choose the use of computer graphics,titling, and/or journaling in the album, I understand that the computer ink used, HP 56 & 57, is labeled �light resistant� up to 108 years. Any possible fading or damage is not the liability of Christina Hall.  Please initial one of the following:  ________Ok to use computer for my project.      ________Do not use computer for my project.

14. I understand that my album is the work of an artist, and changing that work is altering a piece of art. I also understand that altering or adding to an album could cause immediate or future damage to my album and/or photos, and I will not hold Christina Hall accountable for such damage. ____________
