| Assisted Living is for people who want or need: 1. Social interaction and being part of an active community. 2. Assistance with housekeeping. 3. Assistance with laundry. 4. Companionship and social stimulation. 5. Assistance with bathing, grooming, and dressing. 6. Meal preparation. 7. Medication administration or reminders. 8. Monitoring due to forgetfulness or confusion. 9. The security of 24 hour staff monitoring. 10. Are 55 years of age or older. What is needed to enter assisted living? 1. You must have a history and physical done by your physician 30 days before entering the facility or within 72 hours of entering-we provide the forms. 2. An admission form must be filled out that includes next of kin and other important information. 3. Our nurse will do an assessment before you enter to make sure we meet your needs and that you receive the appropriate level of care. 4. Based on this assessment, you will be assigned Level 1, Level 2, or Level 3 care, and will be charged accordingly. You will then be reassessed semi-annually or anytime there is a change to ensure we are continuing to meet your care needs. Financial Information: 1. You may pay privately. 2. You may own an Assisted Living Insurance policy that will pay. 3. The government does provide funding for low-income individuals. This is what is termed Medicaid Waiver Services. We can assist you to contact the proper agency and the application process generally takes 60-120 days. 4. There is a $500.00 security deposit required. What we provide: 1. Registered Nurse Administrator 2. Restaurant dining three times daily plus snacks 3. No concentrated sweets diet, if desired 4. All activities 5. Daily bed making, if needed 6. Daily bathroom cleaning, if needed 7. Daily trash take out, if needed 8. Linen change one time weekly, or more often if needed 9. Laundry one time weekly 10. Housekeeping one time weekly/ kitchen, bathroom, dust, vacuum, mop 11. Whirlpool bath 1-2 times weekly 12. Cable TV 13. Medication Provision 14. Private dining room 15. All utilities 16. Internet service in library 17. TV room 18. Living area with fireplace 19. Transportation to scheduled appointments 20. Secured entrance What you provide: 1. Phone service 2. Room furnishings 3. Towels and linens 4. Personal clothing 5. Incontinence supplies 6. Grooming supplies What we can do for health care services: 1. No extra salt is added to our food and we provide diabetics with alternative food choices. 2. We can help cut up food and open packaging. 3. We can send a tray to a room for a short illness. 4. The resident can use a walker or wheelchair on their own for ambulating. 5. The residents must be able to handle thier own incontinence needs. We can provide reminders for them. 6. We can assist with bath or shower. 7. We can prompt and assist with dressing and grooming. 8. We can provide medications. 9. We can assist with the provision of oxygen and nebulizer treatments. 10. We encourage the insulin dependent diabetic to be responsible for insulin injection and blood sugar monitoring. However, we can assist with these activities. 11. A private home care person can be employed by the resident or family to meet needs that are above our level of care. 12. We can handle some mild difficult behaviors but residents must be cooperative with cares and not endanger themselves, other residents, or staff. 13. We provide scheduled transportation and escort services. What we cannot provide for health care services: 1. We do not provide therapeutic diets. 2. We cannot lift residents. 3. We cannot feed residents. 4. We cannot provide complex nursing services. What would result in need for discharge? 1. The need for complex nursing interventions and services. 2. The need to be lifted, fed, incontinence care, ambulated with more than minimal assistance, excessive behavior problems, or wandering. These items are not all inclusive, as it is impossible to set forth all matters that might pertain to dismissal. We will assist each resident and family to the best of our ability if discharge is needed to a higher level of care. The facility reserves the right to make the final discharge decision. |
