TRANSPORTATION CONSENT
I/We authorize, Michelle Gengenbacher, owner of COLOR ME HAPPY DAYCARE ,
has our permission to take my/our child on walking trips, special excursions, and to nearby park facilities. I/We also authorize the child to ride as a passenger in the vehicle owned by the provider. I/We understand that the child will wear a seatbelt and depending on their age are placed in a car seat. I am willing to leave a child�s car seat for use on field trips, if needed. Children will be transported by a licensed driver with proper auto insurance. I understand that Michelle will inform parents of scheduled field trips in advance, but necessary local trips will be made from time to time.
