As a participant in the OREGON YOUTH SOCCER ASSOCIATION, I agree to abide by all training rules regarding the use of drugs/alcohol/tobacco. Chemical dependency is a progressive, but treatable, disease. Dependency is characterized by continued drinking/drugging, in spite of recurring problems resulting from that use. Therefore, I accept and pledge to abide by the training rules listed in the OYSA Policy Manual, and any others established by my parents or coach.

1. Support my fellow players by setting an example and abstaining from alcohol, drugs and tobacco use.
1. Not enable my fellow players who use drugs, alcohol or tobacco. I will not cover up for them or lie for them if any rules are broken. I will hold my teammates responsible and accountable for their actions.
2. Seek information and assistance in dealing with my own or my fellow players’ problems.
3. Be honest and open with my parents and coach about feelings, needs and problems, especially when the best interests of my fellow players and my team are being jeopardized.
