CCPWFL Coaches Rules for Submitting Mandatory Play Sheets

Completing Mandatory Play Sheet - The following must be filled in on form:

1.

Date of game.

2.

Name of opponent.

3.

Check of the appropriate number of plays required.

4.

Have the opponent (Coach) print name in the Weigh-master, log the reason for missing players, their weight if over, Put in phone number.

5.

Have the opponent print name in the Play Monitor, Phone number.

6.

Final Score EVEN ON MITEY MITE GAMES.

7.

NOTE - DO NOT LET THIS SLIDE OR YOU WILL BE IN VIOLATION OF POP WARNERS MPR RULE.

8.

PLEASE DO NOT JUMP AROUND......EX. PLAY # 1, 6, 9, 10.....THIS DOES NOT HELP.

Mandatory Play Sheet - Instructions for Mailing

1.

All MPR sheets must be generated by the Pop Warner Administrator Software program.

2.

All MPR sheets must be completely filled in (PLEASE REVIEW FORMS BEFORE MAILING).

3.

All MPR sheets must be signed! (PLEASE PRINT NAME SO WE CAN READ)

4.

All MPR sheets must be sent in the SAME envelope, DO NOT FAX THEM.

5.

All MPR sheets must be received (not mailed) by Thursday following your game (including Mighty Mites).

6.

Any Association not following ALL of these rules WILL be fined and sanctioned per occurrence.
