Camp/transportation permission slip

 

 

I give permission for my son, ______________________________(son’s name)

to be transported by personally owned vehicle, to and from a troop camp/activity by, ______________________________________________(driver(s) name).

 

The driver(s) listed may use their best judgment and may seek out any medical attention deemed necessary during transportation of my son. 

 

While at the camp/activity, the Registered adult leaders will be responsible for my son. The adult leader in charge of the camp/activity will secure any medical attention required during the camp/activity.

 

 By signing below I realize that I give such leaders/Drivers my permission to seek out any needed medical attention and will hold them harmless to the cost of such.

 

Any BSA/Tidewater Council accident insurance coverage due to me will be secondary coverage to any and all other personal insurance I carry on my son.

Driver(s) must keep this form during transportation, and maintain this form until the successful completion of the trip.

 

Automotive insurance is the responsibly of the driver and should be maintained at all times.
