The
attempt to find appropriate programs and services for persons with an Autism
Spectrum Disorder can be like finding a path through a maze of detours. As we negotiate the road program, it is
essential that we document all of our efforts and keep these records
up-to-date. The following check list is
designed to help made the road to success easier. Good Luck!
1.
Give
your name.
2.
Ask
for the proper person.
3.
Give
the reason for your call.
4.
If
you cannot reach the person, leave a message with your name, phone number, date
and time and the reason for your call.
5.
Ask
for the name of the person answering your call.
6.
Ask
when your call will be returned.
7.
Maintain
a record of your call.
8.
Call
again (the next day for an emergency), (next week for information).
1.
Be
brief and to the point (one page if possible).
2.
Address
the appropriate person.
3.
Keep
a photocopy for your records.
4.
Send
copies to all parties mentioned or involved.
1.
Obtain
reports from school, medical consultants, therapists, hospitals. It may be necessary to request these reports
in writing.
2.
Keep
a telephone index of the names, addresses.
3.
Prepare
a summary of your child’s developmental milestones, health and medical history,
including immunizations, contagious diseases, allergies, and which services you
have received.
4.
Keep
these documents up-to-date and always keep a copy for yourself. A three-ring binder with tabs is an excellent
way to keep all the information together and organized.
5.
When
negotiating for services with an agency, offer to send them
copies of pertinent information, but do not overload them with written
information that they do not require.
