Nepal
I was lucky enough to join Duncan Chesell Climbing Guide and Everest Summiter on a month long trip in Nepal through October 2001 for a trip dubed the "Three Peaks". Duncan is an excellent guide and has a brilliant team of locals he works with in Nepal including climbing sherpas and sidars and a brillliant cooking team. (Most of the group actually managed to put on weight despite the altitude, the schedule and Nepal's famous gastro bugs). I can thoroughly recommend Duncan as a guide in Nepal. Having said this expect to get pushed as one of the climbers on our trip put it "I loved every minute of it but I'm F#@$ED" but if you go that extra yard with him you will gain great rewards and experiences.
