3 copies of invoice, or proforma invoice, or purchase order
certified copy of certificate of registration, where applicable
manufacturing license, where applicable
special approval forms, where applicable
Note : - maximum number of samples allowed to be imported for type
approval (testing) purposes is two (2) units only.
- application is free of charge.
- incomplete applications will not be accepted.
