101.1 EXPLAIN THE RESPONSIBILITIES OF THE FOLLOWING PERSONNEL AS APPLIED TO SAFETY:
A. CO - ESTABLISH A SAFETY PROGRAM AND PRE-MISHAP PLAN, COORDINATE WITH LOCAL COMMANDS AND MANAGE RELATIONS WITH LOCAL MEDIA AND THE PUBLIC. THE CO SHALL REQUIRE THAT PERSONS CONCERNED ARE INSTRUCTED AND DRILLED IN ALL APPLICABLE SAFETY PRECAUTIONS AND PROCEDURES; THAT THESE ARE COMPLIED WITH AND THAT APPLICABLE SAFETY PRECAUTIONS, OR EXTRACTS THERE FROM, ARE POSTED IN APPROPRIATED PLACES. THE CO WILL ALSO INVESTIGATE AND PROCESS CLAIMS ORIGINATING FROM AIRCRAFT MISHAPS.
B. AVIATION GROUND SAFETY OFFICER - WILL ACT AS PRINCIPLE ADVISOR TO THE CO ON ALL AVIATION SAFETY MATTERS. WILL ASSIST AND ADVISE THE CO IN THE ESTABLISHMENT AND MANAGEMENT OF THE COMMAND AVIATION SAFETY PROGRAM AS PER (REF F). ALSO MAINTAINS APPROPRIATE SAFETY RECORDS, MISHAP STATISTICS AND COORDINATE ALL SAFETY MATTERS AMONG THE ORGANIZATIONS STAFF.
C. DEPARTMENT HEAD - IS RESPONSIBLE FOR SAFE MAINTENANCE, SAFE AND HEALTHFUL WORKING CONDITIONS, STRENGTHENS THE ADMINISTRATION OF THE SAFETY PROGRAM. IN SUPPORT OF THE CO IN HIS SAFETY RESPONSIBILITIES, THE DEPARTMENT HEAD SHALL KEEP THE CO ADVISED ON THE STATUS OF THE SAFETY PROGRAM WITHIN THE DEPARTMENT, PERFORM ACCIDENT PREVENTION FUNCTIONS AS THE CO MAY ASSIGN, ENSURE CORRECTION OF ALL HAZARDOUS CONDITIONS REVEALED BY SAFETY HAZARD REPORTS AND MAINTAIN A RECORD OF ACCIDENT AND SAFETY HAZARD REPORTS.
D. DIVISION OFFICER - ENSURE ALL HANDS MAINTAIN AND FOLLOW SAFETY STANDARDS AND REGULATIONS WITHIN THE WORK SPACE. THE DIVISION OFFICER WILL BE THE SAFETY OFFICER FOR HIS DIVISION. HE SHALL KEEP THE DEPARTMENT HEAD ADVISED ON THE STATUS OF THE UNITS SAFETY PROGRAM WITHIN THE DIVISION, DESIGNATE A SENIOR PETTY OFFICER (E-6 OR ABOVE IF AVAILABLE) AS DIVISION SAFETY PETTY OFFICER, INVESTIGATE DIVISIONAL ACCIDENTS AND NEAR ACCIDENTS, ENSURE THAT CORRECTIVE ACTION IS TAKEN IMMEDIATELY ON HAZARDOUS SITUATIONS REVEALED BY ACCIDENT/HAZARD REPORTS AND ON RECOMMENDATIONS MADE IN ACCIDENT/INJURY REPORTS.
E. SAFETY PETTY OFFICER - SHALL: ENSURE THAT ALL ASSIGNED WORKSPACES ARE INSPECTED AND MAINTAINED FREE OF HAZARDS AND ARE IN COMPLIANCE WITH APPLICABLE NAVOSH STANDARDS. ENSURE THAT ALL ASSIGNED PERSONNEL ARE PROPERLY TRAINED FOR THEIR BILLET, ADVISED OF ANY ASSOCIATED HAZARDS, AND ARE EQUIPPED/PROVIDED WITH APPROIATE PROTECTIVE CLOTHING/EQUIPMENT. TAKE PROMPT ACTION TO ABATE/CORRECT ANT IDENTIFIED DEFICIENCY UNDER THEIR CONTROL. EMPHASIZE SAFETY AS A TOP PRIORITY IN ALL ACTIVITIES CONSISTENT WITH MISSION REQUIREMENTS. ENSURE MISHAPS/NEAR MISHAPS ARE INVESTIGATED AND RESULTS ARE REPORTED TO THE SAFETY OFFICER.
F. ALL HANDS - FAMILIARIZE AND COMPLY WITH SAFETY STANDARDS, SAFETY REGULATIONS AND DIRECTIVES APPLICABLE TO THEM AND TO THEIR ASSIGNED DUTIES. ALSO TO REPORT HAZARDS AND MISHAPS IAW THE COMMANDS AVIATION SAFETY PROGRAM.
101.2 EXPLAIN THE FUNCTIONS OF THE SAFETY COUNCIL/ENLISTED SAFETY COMMITTEE: (REF. C)
ESTABLISHED TO ENHANCE INTERDEPARTMENTAL COMMUNICATION IN ACCIDENT PREVENTION AT THE DIVISION/WORK CENTER LEVELS AND TO ANALYZE PROGRESS OF THE OVERALL SAFETY PROGRAM.
101.3 STATE THE PURPOSE OF THE NAVAL AVIATION SAFETY PROGRAM. (REF. C)
TO PRESERVE HUMAN AND MATERIAL RESOURCES. THE PROGRAM ENHANCES OPERATIONAL READINESS BY PRESERVING THE RESOURCES USED IN ACCOMPLISHING NAVAL AVIATION MISSIONS.
101.4 DEFINE THE FOLLOWING TERMS AS EACH RELATES TO MISHAP INVESTIGATION AND REPORTING: (REF. A)
A. MISHAP - ANY UNPLANNED OR UNEXPECTED EVENT CAUSING PERSONNEL INJURY, OCCUPATIONAL ILLNESS, DEATH, OR MATERIAL LOSS OR DAMAGE, OR AN EXPLOSION OF ANY KIND WHETHER DAMAGE OCCURS OR NOT.
B. NEAR MISHAP - AN ACT OR EVENT IN WHICH INJURY OR DAMAGE WAS AVOIDED MERELY BY CHANCE.
C. REPORTABLE DAMAGE - ALL CASES OF ACCIDENTAL MATERIAL (PROPERTY) DAMAGE INVOLVING A REPAIR/REPLACEMENT COST OF $10,000.00 OR MORE AS A RESULT OF A MISHAP.
D. REPORTABLE INJURY - ANY INJURY, DEATH OR OCCUPATIONAL ILLNESS WHICH RESULTS FORM LOST WORK DAYS, MAN OVERBOARD, ELECTRICAL SHOCK, CHEMICAL OR TOXIC EXPOSURE, EXPLOSIVE MISHAP, OR ALL INJURIES THAT RESULT FROM MOTOR VEHICLE MISHAPS.
E. HAZARD REPORT - USED TO REPORT A HAZARD AND THE REMEDIAL ACTION TAKEN, SO THAT OTHERS MAY TAKE SIMILAR ACTION TO ELIMINATE THE HAZARD; REPORT A HAZARD AND RECOMMENDED CORRECTIVE ACTION BE TAKEN BY ANOTHER ORGANIZATION TO ELIMINATE THE HAZARD; OR REPORT A HAZARD SO THAT SOME OTHER ORGANIZATION MAY DETERMINE APPROPRIATE CORRECTIVE ACTION TO ELIMINATE THE HAZARD.
101.5 STATE THE PURPOSE FOR THE PERSONAL INJURY/OCCUPATIONAL ILLNESS,DEATH, AND MATERIAL DAMAGE REPORTING SYSTEM. (REF. A)
MISHAPS THAT RESULT IN DAMAGE TO NAVY FACILITIES AND EQUIPMENT OR OCCUPATIONAL DEATHS AND ILLNESS TO NAVY PERSONNEL DEGRADE OPERATIONAL READINESS AND INCREASE OPERATIONAL COSTS. INVESTIGATION OF SUCH MISHAPS TO IDENTIFY CAUSES AND PREVENTATIVE ACTIONS, AS WELL AS ESTABLISHING ACCURATE RECORD KEEPING, ARE ESSENTIAL TO THE SUCCESS OF THE NAVY OCCUPATIONAL SAFETY AND HEALTH (KNAVISH) PROGRAM. MISHAP INVESTIGATIONS AIMED AT DETERMINING HOW AND WHY THE EVENT OCCURRED ARE NECESSARY TO PREVENT FUTURE OCCURRENCE OF SIMILAR EVENTS. ACCURATE RECORDS ARE NECESSARY TO ESTABLISH TRENDS, CONDUCT ANALYSES AND TO ASSESS THE EFFECTIVENESS OF THE OVERALL KNAVISH PROGRAM.
101.6 STATE THE GOALS AND MINIMUM REQUIREMENTS OF THE COMPONENTS OF THE FOLLOWING KNAVISH PROGRAM ELEMENTS : (REF. A)
A. HEARING CONSERVATION
1. GOALS: PREVENT OCCUPATIONAL NOISE RELATED HEARING LOSS AMONG NAVY PERSONNEL.
2. MINIMUM REQUIREMENTS: SURVEY OF WORK ENVIRONMENTS TO IDENTIFY POTENTIAL HAZARDOUS NOISE LEVELS AND PERSONNEL AT RISK. ENVIRONMENTS THAT CONTAIN OR EQUIPMENT THAT PRODUCES POTENTIALLY HAZARDOUS NOISE SHOULD, WHENEVER IT IS TECHNOLOGICALLY AND ECONOMICALLY FEASIBLE, BE MODIFIED TO REDUCE THE NOISE LEVELS TO ACCEPTABLE LEVELS. REDUCTION OF NOISE AT THE SOURCE IS IN THE BEST INTREST OF THE NAVY AND ITS PERSONNEL.
B. SIGHT CONSERVATION
1. GOALS: PROVIDE ADEQUATE EYE PROTECTION AT GOVERNMENT EXPENSE. EMPLOYEES SHALL BE REQUIRED TO WEAR APPROPRIATE EYE PROTECTIVE EQUIPMENT WHEN PERFORMING EYE HAZARDOUS OPERATIONS.
2. MINIMUM REQUIREMENTS: A COMPLETE SURVEY OF ALL ACTIVITY WORK AREAS, EQUIPMENT, AND PROCESSES SHALL BE CONDUCTED TO DETERMINE WHICH ARE EYE HAZARDOUS, WHICH PERSONNEL REQUIRE EYE PROTECTION, AND TYPE OF EYE PROTECTION REQUIRED. IN ADDITION TO THE COMMON EYE HAZARDS, SUCH AS FLYING OBJECTS RESULTING FROM CERTAIN CUTTING AND DRILLING OPERATIONS, THE SURVEY SHALL ALSO CONSIDER THE EYE HAZARDS ASSOCIATED WITH EXPOSURES TO VARIOUS FORMS OF ELECTROMAGNETIC RADIATION (E.G., LASER, ULTRAVIOLET, INFRARED, AND MICROWAVE RADIATION).
C. RESPIRATORY PROTECTION
GOALS: OCCUR THROUGH EXPOSURE TO HARMFUL DUSTS, FOGS, FUMES, MISTS, GASES, SMOKE, SPRAYS AND VAPORS. ONLY WHEN ENGINEERING CONTROLS ARE NOT PRACTICAL OR APPLICABLE SHALL PERSONAL RESPIRATORY PROTECTIVE EQUIPMENT BE EMPLOYED TO REDUCE PERSONNEL EXPOSURE.
MINIMUM REQUIREMENTS: EACH ACTIVITY SHALL ASSESS ALL WORKPLACES TO DETERMINE IF THE HAZARDS ARE PRESENT THAT NECESSITATE THE USE OF PPE. IF SUCH HAZARDS ARE PRESENT, OR LIKELY TO BE PRESENT, SELECT AND HAVE EACH AFFECTED EMPLOYEE USE THE TYPES OF PPE THAT WILL PROTECT THE AFFECTED EMPLOYEE FROM THE HAZARDS IDENTIFIED IN THE HAZARD ASSESSMENT. COMMUNICATE SELECTION DECISIONS TO EACH AFFECTED EMPLOYEE. VERIFY THAT THE REQUIRED WORKPLACE HAZARD ASSESSMENT HAS BEEN PREFORMED THROUGH A WRITTEN CERTIFICATION THAT IDENTIFIES THE WORKPLACE EVALUATED.
101.7 STATE THE REQUIREMENTS TO BECOME A GAS-FREE ENGINEER
TECHNICIAN WITHIN AN AVIATION ACTIVITY. (REF. A)
GFE MUST BE QAR/CDQAR AND MUST BE A GRADUATE OF APPROVED AVIATION GFE COURSE. ALSO MUST HAVE A SPECIFIC NUMBER OF OJT HOURS.
101.8 DESCRIBE THE HAZARDS ASSOCIATED WITH RADIO FREQUENCY (RF) ENERGY. (REF. A)
IN ADDITION TO CAUSING BIOLOGICAL CHANGES, RADIO FREQUENCY RADIATION (RFR) CAN GENERATE ELECTRICAL CURRENTS/VOLTAGE LARGE ENOUGH TO CAUSE:
1. LIFE-THREATENING ELECTRICAL SHOCK AND BURNS.
2. PREMATURE (UNWANTED) ACTIVATION (INTERFERENCE) OF ELCTRO-EXPLOSIVES DEVICES (EEDs) IN ORDNANCE.
3. CAUSE SPARKS AND ARCS WHICH MAY IGNITE FLAMMABLE MATERIAL.
101.9 STATE THE MINIMUM AMPERAGE OF ELECTRICAL SHOCK THAT CAN BE FATAL. (REF. D)

8. PROCEDURES FOR INVESTIGATING LASER RADIATION INCIDENTS.
