The purpose of posting student's work online is so that parents and grandparents can have access to your children's work, however I first need your permission and I must inform you that I will never disclose your child's last name or any other personal information. Mrs. Perkins' First Grade Website Photo Parent Permission Form- This website contains the sample permission form that Mrs. Perkins distributes to the parents of her first grade class. The website is not sponsored by the school, rather by Mrs. Perkins herself. She makes herself explicitly clear that her website is not associated with the school, only her first grade class. The website and the form do an excellent job of explaining the status of the website, as well as outlining the ways that the website will still insure the students safety.
Web Site Photo Release Form (Maine)- This link goes directly to a website photo permission form from a Maine school district. It is sponsored by the school district and basically entails what will be posted on the internet or in newsletters. The form has an added caveat though, even if the parent refuses they must do so on this permission form so that the school knows they do not give their permission.
