Differential Diagnosis of all the cases we have discussed in class

·        Mononucleosis

·        Leukemia

·        Polycythemia

·        Microcytic hypochromic anemia

·        Macrocytic anemia

·        Normocytic normochromic annemia

·        Bone marrow neoplasia

 

Case 1

·        57 yr old male

·        marked polychromia

·        moderate microcytosis

·        moderate sperocytes

·        metamyelocytes present

·        polycythemia present--1° due to hypoxia

·        left shift—artificial not attached to infection

·        thrombocytosis

·        Diagnosis—myeloproliferal disorder (infection white cells are only at 35-40)

 

Case 2

·        82 year old male

·        polycythemia

·        with normal wbc and platelet and morphology

·        Diagnosis--2° polycythemia or the patient was dehydrated

 

Case 3

·        29 year old male—history fatigue, infection, back pain, pallor

·        microcytic hypochromic anemia—variable

·        ¯ neutrophils

·        monocytosis

·        throbocytopenia w/ giant forms

·        morphology—promonocytes, nucleated rbc’s, slight hypochromeia

·        Diagnosis—Acute monocytic leukemia made only by the white count.

 

Case 4

·        38 year old male

·        morphology marked microcytosis, ovalocytes, marked aniocytosis &poikilocytosis, marked # of target cells.

·        Next test, TIBC, serum, ferritin

·        Diagnosis thalasemia

 

Case 5

·        Morphology—ovallocytes, aniocytosis, poikilocytosis, macrocytosis, basophilic stippleing

·        Macrocytic anemia—B12 deficiency indicates, Gastrointestinal problems, leukemia, alcohol

·        Thrombocytopenia

·        Leukopenia w/ normal differential, indicates
