Endocrine Case Studies

  1. Johnny, a five-year-old boy, has been growing by leaps and bounds. His height is 100% above normal for his age group, and recently, he has been complaining of headaches and vision problems. A CT scan reveals a large pituitary tumor. A) What hormone is being secreted in excess? B) What name is given to the condition that Johnny will exhibit if corrective measures are not taken? C) What is the probable cause of his headaches and vision problems?
  2. On the morning of February 27, 1991, as Martina sat lazily scanning the morning newspaper, a headline caught her eye – "Anabolic steroids declared a controlled substance." Hmmm, she thought, that’s interesting… it’s about time those drugs got put in the same class with heroin… and then she continued her browse through the paper. That night, Martina awoke from a dream in a cold sweat. In her dream, it seemed that all her male friends and colleagues were being rounded up by government drug agents and charged with illegal possession of a controlled substance. What is the connection, if any, between the morning headline and Martina’s bizarre dream?
  3. Mary Morgan has just been brought into the emergency room of City General Hospital. She is perspiring profusely and is breathing rapidly and irregularly. Her breath smells like acetone (sweet and fruity), and her blood sugar tests out at 650mg per 100ml of blood. She is in acidosis. What hormone drugs should be administered and why?
  4. Mr. Gutteman, a 70-year-old male, was brought into the ER in a comatose state and has yet to come out of it. It is obvious that he suffered severe head trauma – his scalp was badly lacerated, and he has an impacted skull fracture. His initial lab tests (blood and urine) were within normal limits. His fracture was repaired and the following orders (among others) were given :
  1. Explain the rationale behind these orders.

On the second day, the hospital aide reports that Mr. Gutteman is breathing irregularly, his skin is dry and flaccid, and that she has emptied his urine reservoir several times during the day. Upon receiving this information, the physician ordered :

Mr. Gutteman is found to be losing huge amounts of water in urine and the volume lost is being routinely replaced (via IV line). Mr. Gutteman’s blood and urine tests are negative for sugar and ketone.

b) What would you say Mr. Gutteman’s hormonal problem is and what do you think caused it?

c) Is this condition life threatening? Explain.
