Kingston General Hospital, Kingston (liaison pharmacist responsible for enforcing if > 24 hours ordered)
The Hospital For Sick Children, Toronto (recommended in Antimicrobial Guidelines, order sets. Not enforced. Most procedures < or = 24 hours

3: Eprex is given to patients prior to, or after major surgery at:
