Presenting complaint is written in Chronological Order i.e., the symptoms occurring 1st ll be written 1st with duration of each Symptom. Also DM and HTN will be asked b/c these diseases are not fully curable so a patient, whenever visits Doctor with any Presenting Complaint, these two diseases will also be associated.
