• Include SASE if you require Dinner & Registration confirmation, otherwise none will be sent. Registration packets will NOT be mailed, pick up upon arrival. Be sure to include your full name and the full name of anyone traveling with you, one form per person if not same address. No refunds will be made after 1 October 2002.

  Important Notes:

• The Georgia-Flordia Football Game is being held in Jacksonville, FL the same weekend as the Memorial Service. Make all of your reservations as soon as possible. If you wait until Oct.-Nov you may not get a room.
• Auditorium Seating for the Memorial Service is limited to the 1st 1,000 registered. Base passes will not be issued this year.
• Hospitality Hotel is the Ramada Inn, 912 729-3000, $42.00 per night plus tax. (right accross from Howard Johnson's & Cracker Barrel.) When making reservations make sure to tell them you are with Subvets to the rate they have offered. Call the Sub Museum, 912 882-2786 ASUB), for other lodging options in the area.
• Daily 50/50 and Prize Raffles will be held in the Hospitality Room.

• SSBN & TTF TOURS - Due to world events, all tours are subject to cancellation, without notice. There are 5 SSBN tours scheduled to include 35 people (175 slots), plus a representative and 2 TTF tours. If there is an overwhelming reponse to SSBN tours, a lottery will be held, by the Sub Museum Manager, for Subase, determining who gets to go and when they get to go. (It may not be during the time slot you selected)

  Each person who wishes to go on either of these tours: MUST FILL OUT THE PRIVACY ACT RELEASE FORM. THE FORM MUST INCLUDE YOUR SUBVET WWII MEMBERSHIP NUMBER. BE SURE TO INCLUDE THIS NUMBER ON YOUR GUESTS FORM, AS WELL. IT IS PART OF THE VERIFICATION SYSTEM. THIS IS MANDATORY, PER THE SUBASE.

• A MILITARY ID CARD WILL NOT GET YOU ON THESE TOURS.
• DO NOT BRING YOUR PRIVACY ACT RELEASE FORM WITH YOU. IT MUST BE SUBMITTED TO THE SUBASE, VIA THE SUB MUSEUM by mail, BEFORE YOU ARRIVE.
• TO GO ON THESE TOURS YOU MUST BE ABLE TO WALK, CLIMB STAIRS AND / OR LADDERS UNASSISTED FOR 2 HOURS.
• YOU MUST BE AN AMERICAN CITIZEN TO GO ON THESE TOURS.
